Junior High Sports
Students participating in interscholastic athletic programs, including intramural sports, shall have an annual physical examination prior to tryouts and participating in any scheduled practice or contest. Physicals are effective for 395 days from the date of the exam. The physical exam form for District 54 can be found by clicking on

Physical Examinations
State law and district board policy requires students to have a current physical examination and all required immunizations on file with the school nurse in order for a student to try out and/or participate in interscholastic sports or intramurals. The exam must have been administered within one year prior to starting that grade level. The exam must be documented on the
Certificate of Child Health Examination form that can be accessed by clicking here and is the only acceptable form for the physical exam. The medical provider must complete the physical exam and sign and date the medical portion of the form. Parents are required to complete the heading (name, address, etc) and complete and sign the Health History section of the form. Incomplete forms will not be accepted. Current physicals are effective for 395 days from the date of the examination.
